Different Form Types
"Online" Web Forms
If you can't complete the web form online, then you can complete it offline and email it to us with any other required information. This will get to us faster than by post and you will also get a confirmation of delivery email once it is delivered to the RST Inbox.
Download and Save the Web Form
- Click on the form you require.
- The form should be displayed in your browser.
- You can either:
- Complete the fields and then print, or
- Leave the fields blank and manually fill them in, then
- Press the Print button at the bottom of the form and the Print dialogue is displayed
- You can then:
- Save a copy for yourself so that you can email to us later by:
- Select that you want to Save the document as a PDF (do not print), and
- If necessary, select the file destination
- Save the file on your local machine
- Save a copy for yourself so that you can email to us later by:
- If you have completed the form, then:
- Attach it to your email and sendto us, or
- Open the file as a PDF and print it off to complete manually on screen, save and email to us, or
- Print it off to complete manually, complete it, scan and save and then email it to us
Standard Word Forms
The standard forms can be completed on word processor software (such as Word or Pages) or as PDF documents. Once completed you can email them to us with any other required information. This will get to us faster than by post and you will also get a confirmation of delivery email once it is delivered to the RST Inbox.
Download and Save the Standard Word Form
- Click on the form you require.
- The form should be opened in Word /Pages or similar application. If it is, then,
- Complete the form and save the file on your local machine
- Please save as a standard format file such as DOC, DOCX or PDF
If the form does not open on your machine, it may be because you do not have compatible software. If this happens, you can save the file using Print and Save as a PDF on your browser and then complete the information.
Send Email with Subject
Once the form is complete you can email us (except active employees of Non-TOCs, Network Rail and BTP see below), attaching the form and any other required files/information.
Use the following links to add a default Subject into the email that you are sending us. Please add you forename and surname after the default subject text.

Once an email has been sent you will get a confirmation of delivery email once it is delivered to the RST Inbox. If you do not receive an automatic reply from RST, then we probably haven't received your email.
Active Employees of Non-TOCs, Network Rail and BTP
Please contact your usual travel facilities contact point as below:
- Active non-TOC staff - forms MUST be sent to your usual travel facilities contact point. In most cases, this will be via your employer.
- Network Rail staff - forms MUST go via the Safeguarded Helpdesk
- BTP staff - forms MUST go via the HR Business Centre
